Social determinants of health are nonmedical factors that can affect a person's health. These factors include where someone lives, household income, access to healthy food and availability of transportation.

- Money: Financial help can make it easier to pay for treatments and medicines.
- Transportation: If you live far from a hospital or don't have a car, services such as medical transportation or community ride-sharing can help you get to healthcare appointments.
- Food: Access to affordable, healthy food can be improved through local food banks, meal delivery services and community gardens.
